One thing that's always slightly bothered me is how friendly fire is never given the attention it demands. I've never encountered friendly fire casualty numbers in connection with wars/battles before (without explicitly searching for them). Oftentimes, they easily make up for 10 - 20% of the total casualties.


Light novels, especially those with 1000+ chapters, tend to have extremely oversized battles. But they never seem to acknowledge the existence of friendly fire.
