In terms of hierarchy, I have never actually seen a Vampire King before–the Lords themselves were all too busy squabbling with one another to actually consider bowing to another one of them, even if the person was deemed stronger.
However, the lycans could choose a King and respected the strength.
There were fair and equal methods of usurping power from the Lycan King and if you did something like poison or a back attack–that would not gain you the favor needed to be king.
