Brother Dearest

Ivy knew it would eventually happen. Shes on Derek's service and now am forced to spend time with him and Yang. Ivy finds them in the hall and is just in time to hear Derek boast, "You need a God. Neuro God right here."

"Godly? Your entire day consists of putting shunts in people," Christina deadpans.

Ivy snorts, "You know, I'm glad someone else is knocking you down a little."

He smiles sarcastically at her, "Today? Temporal Lobectomy."

They hear Webber presenting something, "-Hospitals across the country have reported substantial savings. And to make sure this was the right decision, I have enlisted one of our own to do a trial run for the last month. Dr. Sloan."

Ivy leans towards Derek, "How much do you want to bet he makes this more about himself than the actual computer?"

"Nothing, cause you will win," he whispers.

Mark leans against the board, "I know what you're all thinking. Its a machine. It doesn't know me. It doesn't know what I like. But after using this new system for the past month, I gotta say, people. I'm a convert. This computer, knows me better than I know me."

Derek moves forward throughout the presentation as she roll her eyes at the two of them.

"Chief, this thing has rearranged my schedule for the entire week. I mean haven't we had enough change here already?" Derek turns to us.

Chief ignores Derek, "Change is good, everyone. Embrace it."

As everyone disperses, Bailey and Ivy walk towards Derek, "You two need to stop bickering. Its unattractive."

"Yes and we all know how important it is for Derek to look good,' Ivy smirks at him.

He makes a face at her. "Haha,"

Cristina and Ivy head to the residents locker room, she goes to talk to Karev, while Ivy goes to Reed.

"Heard you were in the O.R. all night. Anything good?" She asks, sitting on the bench.

Reed stretches, "Hang Glider. Needed their intestines sewn back together."

"Awesome, guess what. I get the pleasure of working with brother dearest today, and I can't go complain to Jackson or Charles because they will try and make it seem like a good thing. So can you be moody and mocky with me?" Ivy pleads with her.

"Oh, fun. Whole day with the greatest Neuro surgeon in the U.S. Oh how lucky you are my dear friend," Reed mocks, making Ivy laugh.

She shakes her head, "Thank you, this is why you are my favorite friend."

Ivy sees Cristina leave and heads out after her.

Derek paged Ivy to the scan room. They were looking over an MRI scan when someone came in.

"Dr. Shepherd," naturally, both look up.

Derek obviously recognizes him, "Isaac, if you don't stop calling me Dr. Shepherd, especially now that two of us work here, I'm seriously going to have to cut you off."

Ivy waves in a friendly greeting, "Right. Forgot. Derek and Ivy, right?" She nods.

"Thanks."

"I have your scans Derek," Isaac starts to open.

"I didn't order any scans," Derek denies.

Isaac is pushy, "I think they're for a consult, but I can come back if you two are busy."

"No, its okay," Derek tells him.

Isaac takes down a scan and puts the one he has up. Its a beautiful tumor.

"Wow," Ivy says.

Derek sighs, "Oh, its a tumor on the spinal cord. Probably a hemangioblastoma."

"You've seen one before?" Ivy asks him.

He shakes hisbhead at her, "Not this big. Teaching moment, you see how the tumor has worked its way up the spine? Its taken over the blood supply. Its a perfect tumor."

Ivy looks at Isaac, "Does the patient still have leg function?"

Derek looks at her in surprise, "What? You forget that I went to medical school and I listen to you rattle on about this kind of stuff for years."

Isaac answers her original question, "Yes."

"Incredible," the Shepherds say together.

Derek goes on, "He shouldn't be walking. He should be paralyzed, dead."

Isaac gets a strange look on his face, then quickly clears it, "You'd have to operate quickly, then, to take the tumor out?"

Derek has a sort of dreamy look on his face, "Oh, I'd love to operate on this. But, no, you have to cut the cord, paralyze the patient, to kill the tumor. Who's the doctor on this one?"

Derek turns towards Isaac, "You."

Ivy looks up quickly, "What?"

"These are my scans. The tumor is mine. I'd like you to take it out. I'd like you to try."

Derek and Ivy look back up at the scans. Ivy grew excited at the idea to scrub in on such a beautiful tumor, while Derek was trying to see if it was worth the risk or even possible.

"One doctor said it was bad circulation, another one, pinched nerve. But when the pain started to interfere with work, I decided to run an M.R.I on myself," Isaac explains to the duo.

They're pagers then start to go off, "Isaac, had I known this was your scan, I wouldn't have-"

"I have been to every doctor, Derek. Every time. Its the same thing. They look at the tumor and they're defeated. But not you. You were inspired. That's why I know I was right to come and work here. I needed to meet you," Isaac tells.

Derek focuses on the last part, "You got a job here to meet me?"

"I have researched every neurosurgeon in this country. I know their statistics, I know who wins the most prizes, but I also know who takes the most risks. That's you," Isaac tells him.

Ivy speaks up, "Derek, you know how much I hate feeding your ego, but Issac is right. You can do this. You may be the only one who can and look at the beautiful tumor, you will be able to do this."

"I know you guys believe I can take this tumor out, and I wish I could. But, Isaac, this surgery will kill you," Derek tells him, seriously.

Isaac doesn't give up, "Take the scans. Look them over. But stop your mind before the part where it gets to 'no'. Think yes, yes?"
