However, when Leon Hayden reached the section for the guardian's information, his hand hesitated with the pen, as if pondering what to write in "relationship to patient."
After pausing for a moment, he continued to fill out the form and then handed the completed document to the medical staff he had spoken to just before, preparing to leave.

Just as he was about to turn away, the medical staffer suddenly called out to him.
Leon Hayden turned around, looking puzzled at the lady nurse.
"Mister, did you fill this out incorrectly?" The nurse pointed to two distinctly different names on the form, thinking he had made a mistake.
Leon Hayden walked back, looked down at where the nurse was pointing, and then, lifting his gaze, replied, "No."
Hearing this, the nurse seemed a bit taken aback.
"But isn't the little girl your daughter?" The nurse was confused. One of the names was 'Howard' and the other was 'Harrison', and the relationship section even listed 'friend'.
