Itsuki didn't just tell her the truth, he showed it to her.
Itsuki showed Nagisa his connection with Yoshino in the other timeline. When her memories returned, she went to find him.

Yoshino felt like she owed her mother a lot.

AN: So, Nagisa is in love with Itsuki, but Itsuki doesn't know that. He thinks she resents him for what happened years ago. He believes he had hurt her when she was vulnerable, and now that he was in front of her being in a relationship with her daughter, he could feel that he had hurt her once again.
It's like seeing your scummy ex get together with your daughter and coming to you to ask for your daughter's hand in marriage. Itsuki believes she has every right to be pissed at him, and he thinks she must really hate him now, and he regrets getting too close to her as this has complicated the relationship because he can't just leave Yoshino. After all, this time, he won't leave anyone behind.
So, even he knows his apology is hypocritical , because he won't stay away from Yoshino as long as she doesn't want him to stay away. The only thing he regrets is getting romantically involved with Nagisa who was Yoshino's mother, as he believes she would have been better off not meeting him at all.
This sentiment hurt Nagisa, as she felt Itsuki considered the thing between them a mistake while Nagisa did not.
Itsuki isn't at fault here because Nagisa was the complicated one, and Itsuki usually doesn't use his empath abilities when dealing with people.
While Nagisa acts like she resents him, and maybe she thinks she dislikes him, but she actually does not, at least not for the reason she thinks she does.
She thinks she dislikes him for being a womaniser, but she actually dislikes him for walking away from her without even trying.
Itsuki acted very normally and treated Nagisa like a normal friend and employee as he felt he was doing the right thing, but Nagisa felt only more aggrieved as she felt, to Itsuki, that their moment meant nothing.
It's like when someone tells their boyfriend or girlfriend that they want to break up, and they just say "okay" nonchalantly instead of acting up. It makes the other party feel like they didn't give a shit about the relationship, and the one who initiated the breakup feels bitter. Usually, it happens more commonly with women that doesn't mean there aren't petty men.
If he had pushed a bit more, then she would have folded, but Itsuki didn't know that. All he saw was Nagisa's tears and eyes filled with anger, betrayal and hurt.
