Ice

Most people don't understand how extremely difficult it can be to find an opening in ice from underwater. This has naturally lead to many unnecessary deaths.

The main issue, is that air trapped under ice will appear as if it were an opening. In connection with how air leaves your mouth whilst breathing out, you'll have numerous "fake exits" in no time.

The obvious solution is to never go swimming under ice.
